Additional Collection Instructions	Collect 1 x 5mL serum plain (red) tube and 1 x 4 mL EDTA (purple) tube. Please fill in a Transfusion Reaction Form available from the Blood Bank laboratory. Please also send the implicated blood pack including attached giving set, to the laboratory. Handwrite patient details on the tube and sign your name on the request form in the collection declaration section. If a pre-printed label is used on the specimen, it MUST bear the collector's full signature and time/date of collection.
