Tests on the Source:  Hepatitis C antibodyHepatitis B surface antigenHIV antibody, plasma stored for Hepatitis C Viral Load and serum stored.

The requesting doctor must get consent from the patient prior to collecting blood for HIV testing.  The requesting doctor must sign the request form before HIV testing is done.
